About this policy
This policy explains how pgt.clinic handles personal information, including health and genetic information. pgt.clinic is a clinical and reproductive genetics practice operated by Monash IVF Pty Ltd, ABN 63 006 942 990. We are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and by state health records laws where they apply, including the Health Records Act 2001 (Vic).
What information we collect
To provide a genetics consultation we collect:
your name, date of birth, contact details, Medicare and private health insurance details
your referral, medical history, examination findings and test results
your family history, which may include information about your relatives
genetic test results, including results for relatives when you or they provide them
records of our consultations, letters and communications with you and your doctors
If you contact us through the website, we collect what you enter in the contact form: your name, email address, phone number if given, and your message.
How we collect it
We collect most information from you directly, at consultations and in forms you complete. We also collect it from your referring doctor, from other health professionals involved in your care, from laboratories that perform your tests, and, with your agreement, from relatives or their health professionals. Please don't send health information through our website contact form, which isn't a secure channel.
Why we collect, use and share it
We use your information to assess you, arrange and interpret genetic tests, give you advice, and keep your referring doctor and other treating doctors informed. We also use it for billing, Medicare and health fund claims, practice administration, and to meet our legal and professional obligations.
We share it with:
your referring doctor and other doctors involved in your care
pathology laboratories that perform your tests
Medicare and your health fund, for billing
My Health Record, unless you ask us not to upload
our service providers, such as our practice software, telehealth and IT providers, who are bound to keep it confidential
We don't sell personal information or use it for marketing.
Genetic information and your relatives
Genetic results can matter for your blood relatives. We will talk with you about who in your family may benefit from knowing a result and how to share it, and we can provide a letter for relatives to take to their own doctor.
In rare cases the Privacy Act allows a health practitioner to share genetic information with a genetic relative without consent, where they believe it is necessary to lessen or prevent a serious threat to that relative's life, health or safety and they follow guidelines approved by the National Health and Medical Research Council. We would discuss this with you first wherever possible.
Research
We won't use information that identifies you for research without your consent, unless the research has been approved by a human research ethics committee in line with the law.
Overseas disclosure
Some genetic tests are only available from laboratories overseas, most often in the United States, the United Kingdom or Europe. When a test is sent overseas, we tell you beforehand and include it in your consent. Some of our service providers may also store information outside Australia, including our website provider, Squarespace, which stores contact form submissions in the United States.
How we store and protect it
We keep health records in TrakGene, with access limited to the people who need it to provide your care. We take reasonable steps to protect information from misuse, loss and unauthorised access, and we use Zoom for video consultations.
We keep records for as long as the law requires, generally at least seven years after your last contact with us, or until age 25 for someone seen as a child. We then destroy or de-identify them securely.
Our website and the consult guides
The website uses cookies through Squarespace to make the site work and to measure visits in aggregate. You can turn cookies off in your browser settings.
The consult guides for health professionals run entirely in your browser. Nothing entered into a guide is saved or sent to us or anyone else, and it clears when the page is closed.
Accessing and correcting your information
You can ask for a copy of the information we hold about you, or ask us to correct it, by contacting us using the details below. We will respond within 30 days. We may charge a reasonable fee for copying, and we will tell you before we do. If we decline a request, we will tell you why in writing.
Using a pseudonym
You can make a general enquiry without giving your name. We need to identify you to provide a consultation, arrange testing or claim Medicare.
Complaints
If you have a concern about how we have handled your information, please contact us first. We will acknowledge your complaint within 7 days and aim to resolve it within 30 days.
If you are not satisfied, you can contact:
